Normally CPUState::cpu_index is used to pick the right CPU for various
operations. However default consecutive numbering does not always work
for POWERPC.
These indexes are reflected in /proc/device-tree/cpus/PowerPC,POWER7@XX
and used to call KVM VCPU's ioctls. In order to achieve this,
kvmppc_fixup_cpu() was introduced. Roughly speaking, it multiplies
cpu_index by the number of threads per core.
This approach has disadvantages such as:
1. NUMA configuration stays broken after the fixup;
2. CPU-targeted commands from the QEMU Monitor do not work properly as
CPU indexes have been fixed and there is no clear way for the user to
know what the new CPU indexes are.
This introduces a @cpu_dt_id field in the CPUPPCState struct which
is initialized from @cpu_index by default and can be fixed later
to meet the device tree requirements.
This adds an API to handle @cpu_dt_id.
This removes kvmppc_fixup_cpu() as it is not more needed, @cpu_dt_id
is calculated in ppc_cpu_realize().
This will be used later in machine code.

This is an autogenerated patch using scripts/switch-timer-api.

Since current_cpu is CPUState it no longer depends on CPUPPCState.
Move ppce500_set_mpic_proxy() to a new hw/ppc/ppc_e500.h because
hw/ppc/ppc.h is too heavily using CPUPPCState and PowerPCCPU.

Move next_cpu from CPU_COMMON to CPUState.
Move first_cpu variable to qom/cpu.h.
gdbstub needs to use CPUState::env_ptr for now.
cpu_copy() no longer needs to save and restore cpu_next.

Many of these should be cleaned up with proper qdev-/QOM-ification.
Right now there are many catch-all headers in include/hw/ARCH depending
on cpu.h, and this makes it necessary to compile these files per-target.
However, fixing this does not belong in these patches.

Move it to qom/cpu.c to avoid build failures depending on include order
of cpu-qom.h and exec/cpu-all.h.
Change opaques of various ..._irq_handler() functions to the
appropriate CPU type to facilitate using cpu_reset_interrupt().
Fix Coding Style issues while at it (missing braces, indentation).

Both fields are used in VMState, thus need to be moved together.
Explicitly zero them on reset since they were located before
breakpoints.
Pass PowerPCCPU to kvmppc_handle_halt().
